Forehead Reduction vs. Forehead Lift: Differences and the Benefits of Combining Them

2026-03-11

Hello! This is Respect Plastic Surgery.

The forehead is one of the key areas that determine the first impression of a face. If the forehead appears too wide or saggy, it can make you look older than your actual age or make your facial proportions seem elongated. Because of these concerns, Forehead Reduction and Forehead Lift are the two procedures most frequently asked about during consultations.

While both surgeries aim to improve the forehead line, they differ in purpose and method. Depending on your specific condition, you may choose one or even undergo both simultaneously for optimal results. In this post, we will explain each procedure, who they are recommended for, the recovery period, and the advantages of combining them.

1. How is Forehead Reduction Performed?

Forehead reduction is a surgery designed to reduce a wide forehead and create a balanced facial proportion.

  • Incisional Forehead Reduction: An incision is made along the hairline, and the scalp is pulled forward to reduce the length of the forehead. While it can significantly reduce the forehead in one go, the amount of reduction may be limited by the elasticity of the scalp.
  • Non-Incisional Method (Hair Transplant): This involves designing a new hairline further forward through hair transplantation to make the forehead appear narrower. It has the advantage of having almost no incisions, but costs can increase if a large number of follicles are required.
  •

Our Special "Implant Forehead Reduction"

At our clinic, we perform the "Implant Forehead Reduction," which combines the strengths of both incisional reduction and hair transplantation.

  • Natural Hairline: It reduces the basic forehead length via incision while simultaneously transplanting hair along the front hairline to create a more natural finish.
  • No Posterior Scarring: Unlike typical hair transplants that harvest follicles by incising the back of the head (occipital region), we harvest what is needed from near the front hairline.
  • Minimized Damage: This prevents scarring on the back of the head, minimizes damage to the occipital area, and allows for more delicate hairline adjustments.

2. How is a Forehead Lift Performed?

A forehead lift is a lifting surgery that raises a sagging forehead and drooping eyebrows.

-Purpose: As we age, the forehead skin and eyebrows descend, which can make the eyes look heavy or the overall expression look tired.
-Effect: This procedure improves sagging to create clearer eyes and a more youthful appearance.
-Method: An incision is typically made along the hairline or inside the scalp to pull and fix the sagging forehead tissue upward.
This also helps alleviate deep forehead wrinkles.

3. Recommended Candidates

Forehead ReductionForehead Lift
Wide forehead making the face look longDrooping eyebrows/forehead causing a tired look
Receding or high hairlineDeep forehead wrinkles
Seeking more balanced facial proportionsUsing forehead muscles heavily to open eyes

4. Recovery Period

While individual recovery varies, the general process is as follows:

-Stitch Removal: Approximately 7–10 days.
-Swelling Subsides: Roughly 2–3 weeks.
-Natural Results: Approximately 1–3 months.Initial swelling or a "pulling" sensation is normal and will gradually disappear over time.

5. Benefits of Combining Both Procedures

For those facing both width and sagging issues, combining the two surgeries offers several advantages:

-Simultaneous Improvement: Addresses both forehead length and sagging at once.

-Synergy: Achieves both facial proportion correction and a lifting effect.

-Efficiency: Solves two problems with a single surgery and one recovery period.

-Natural Finish: Results in a more balanced and natural hairline.

6. Actual Patient Case

This case features an actual patient of Respect Plastic Surgery who provided conset for the use of their content in compliance with medical laws.
-Treatment
Since reducing the forehead length alone would not sufficiently improve the eye area, we performed both a forehead reduction and a forehead lift.
-Result
The forehead proportion improved as the hairline was naturally lowered.
The eyes became clearer as the sagging eyebrows were lifted.
The overall impression became brighter and more youthful.
The patient expressed high satisfaction, noting that their eyes felt "much more refreshed."

While forehead reduction and forehead lift surgery have different primary objectives, both play a critical role in improving facial proportions and overall impression. It is essential to develop a surgical plan by comprehensively considering an individual’s forehead length, scalp elasticity, and eyebrow position. In some cases, performing both procedures together can lead to even more balanced and harmonious results. Therefore, the most important step is to determine the method that best fits your needs through a thorough consultationThank you for reading, and we will return with more helpful information in our next post!
